Evidence ladder
How far up the ladder this claim has climbed. A high consensus on a low rung means "consistent so far," not "proven in people."
Top evidence so far: All trials, pooled (Meta-analysis)
How the studies fall
What the evidence shows
The Norwitz-cited 'ezetimibe protects the brain' signal: a real 14-3-3/hexokinase autophagy mechanism (worm/cell) PLUS a retrospective '7x lower dementia' association - almost certainly inflated by healthy-user/indication confounding; a 2026 review even hints at HIGHER cognitive risk. Net: intriguing hypothesis, NOT a finding. Do not rely on it.
The evidence (10)
| Source | Grade | Stance | Quality | Finding |
|---|---|---|---|---|
| n/a 2007 ClinicalTrials.gov | RCT | tested-null | low | No completed ezetimibe RCT for AD/dementia exists (only a terminated pitavastatin study); AD claim rests solely on MR + in-silico/EHR |
| Nordestgaard 2025 · Alzheimers Dement | observational | mixed | moderate | [FT-verified] FT: NPC1L1 MR protective for all-cause/vascular dementia (OR0.18) BUT AD-specific NULL on LDL-exposure sensitivity |
| Williams 2020 2020 · Ann Neurol | observational | tested-null | moderate | [FT-verified] MR: NPC1L1/HMGCR/APOB do NOT affect AD risk; null for ezetimibe-on-AD |
| Gyimesi G 2025 · Physiol Rev | mechanism | contradicts | low | [null on reduces-claim = contradicts] Broad review of the entire SLC transporter superfamily (464 SLCs); mentions neurodegenerative disorders only generically among diseases linked to SLC dysfunction, with no ezetimibe/NPC1L1/AD-specific da |
| Geifman 2017 · Alzheimers Res Ther | observational | tested-null | low | Statin exposure linked to lower AD risk in subgroups but general-population trials null |
| Klionsky DJ 2021 · Autophagy | mechanism | contradicts | low | [null on reduces-claim = contradicts] Methodological guidelines for autophagy assays; no content on ezetimibe or Alzheimer's disease. |
| Jiménez-Sánchez A 2022 · Nutrients | observational | tested-null | low | [null on reduces-claim = contradicts] Narrative review of extra virgin olive oil's cardiovascular/metabolic benefits; no mention of ezetimibe, cholesterol absorption inhibitors, or Alzheimer's/dementia outcomes. |
| Ganne A, et al. 2024 · Aging Biol | mechanism | supports | low | [FT-verified] FT: proteomics+in-silico docking+retrospective EHR not outcome study; 'sevenfold' is hypothesis-grade, title overstates |
| Waters 2010 · J Cardiol | RCT | contradicts | moderate | Statin RCTs did not slow early Alzheimer's; cholesterol-lowering AD benefit not demonstrated |
| McGuinness 2016 · Cochrane Database Syst Rev | meta-analysis | contradicts | high | Cochrane: statins in late life do not prevent dementia/cognitive decline (indirect to ezetimibe) |
Disagree, or know a study we missed?
We grade by evidence, not opinions. The way to weigh in is to point us to a study we haven't cited (check the evidence table above first), or to flag a problem with one we have. Every submission is reviewed; if it holds up, the grade updates and shows in Science Changes Its Mind.
Opens a short form. You'll sign in with Google so submissions are tied to a real account — we don't display your identity, and we only accept a link we can verify (PubMed, DOI, ClinicalTrials.gov).
