What the evidence shows

Handling receipts measurably gets bisphenol A (or, on newer paper, bisphenol S) into you: a five-second hold transfers about a microgram to dry fingers and far more to wet ones, dermal BPA clears over days rather than hours and circulates more often in its unconjugated form, and cashiers carry roughly two to three times the urinary BPA of controls.

observational mixed moderate Narrative review of developers and replacements. Zero weight; the primaries it summarises are appraised here directly. 'The exposure to BPA via dermal transfer has been recently discussed as a significant contribution to the overall human exposure and the estimated daily intake (EDI) has been reported up to 218 μg d-1.'

in-vitro mixed low Nigerian receipts. The abstract's 'not a serious health risk' compares against a 50 µg/kg/day reference value that is not EFSA's current figure. 'The estimated mean daily intakes of BPA by dermal absorption due to handling of thermal receipts were 0.20 and 9.89 μg/day for the general population and the occupationally exposed individuals, respectively'

observational mixed moderate Population-level corroboration of the occupational exposure; the association held after creatinine, race, BMI and age, and was absent in males. 'females with potential occupational exposure to thermal paper receipts have significantly higher levels of urinary BPA excretion (geometric mean (GM): 5.45 μg/L, 95% CI: (4.02, 7.39)) compared to females with unlikely occupational exposure (GM: 2.16 μg/L, 95% CI: (1.73, 2.70)).'

observational mixed moderate Risk-characterisation exercise comparing modelled dermal BPS exposure with DNELs; the two scenarios disagree by 17-fold on workers. An assessment, not a measurement. 'In the second scenario, however, the RCR suggests that the risk might not be adequately controlled for both the general population and workers.'

observational mixed low TEMPERED to mixed: six male volunteers, and the 'outcome' is urinary 8-OHdG, an oxidative-DNA-damage biomarker, not a clinical endpoint. A surrogate moving in six people is mechanism-grade support for harm, not outcome evidence; counting it as `supports` at observational tier would let a biomarker in six people stand in for a human health effect. The exposure half (3x urinary BPA, 51.9-84% of urinary BPA attributable to dermal contact) is solid and belongs to the exposure claim. 'after handling the thermal receipts, the urinary BPA concentrations of volunteers increased 3 times of those before the experimental period' 'There existed a correlation between BPA exposure and 8-OHdG (R2=0.237, p<0.001)'

observational mixed moderate Three monitoring campaigns around the 2020 Swiss ban of both BPA and BPS. BPA fell; BPS rose despite the ban; Pergafast 201 became the majority developer. 'the use of BPA as color developer dropped from 82.2% in 2013/14 to 10.8% in 2021, the fraction of BPS-based thermal paper rose from 3.1% to 19.1% during the same period, despite being banned.'

observational mixed moderate NTP-affiliated. Within-shift BPA change was inconsistent; the between-group difference held. Serum detection was rare — relevant to how much reaches circulation. 'The mean urine BPA concentrations in 31 cashiers who handled BPA receipts were as likely to decrease as to increase after a shift, but the mean post-shift concentrations were significantly higher than those in non-cashiers.' 'Only a few cashiers had detectable levels of total BPA or BPS in serum'

observational mixed moderate 90 cashiers vs 44 controls. Total BPA roughly 2.5x; free (bioactive) BPA not significantly raised — a detail that cuts against the harm narrative and is usually omitted. 'The median urinary total BPA concentration was 3.54 µg/L (2.89 µg/g creatinine) for controls and 8.92 µg/L (6.76 µg/g creatinine) for cashiers.' 'However, no significant increase was observed in urinary free BPA concentration.'

in-vitro mixed moderate The foundational transfer measurement. Exposure premise, not harm. 'When taking hold of a receipt consisting of thermal printing paper for 5 s, roughly 1 microg BPA (0.2-6 microg) was transferred to the forefinger and the middle finger if the skin was rather dry and about ten times more if these fingers were wet or very greasy.'

in-vitro mixed low Brazilian receipts with the highest reported content; occupational modelled intake ~50x the general population. 'Fifth percentile, median, and 95th percentile daily intakes by the general population were 0.44, 1.42, and 2 μg/d, respectively, whereas the corresponding values for occupationally exposed population are 21.8, 71 and 101 μg/d.'

observational mixed moderate The highest human blood levels in this literature, produced by sanitizer + receipt + eating. The abstract's closing sentence links these levels to disease risk by citation, not by measurement; that sentence is not evidence on this claim. 'the combination of dermal and oral BPA absorption led to a rapid and dramatic average maximum increase (Cmax) in unconjugated (bioactive) BPA of ∼7 ng/mL in serum and ∼20 µg total BPA/g creatinine in urine within 90 min.'

observational mixed moderate Same lab and largely the same participants as the 2017 dermal study (one group_id). Substitution context. 'Both the in vitro and human studies suggested lower percutaneous absorption of BPS compared with BPA, but a lower biotransformation efficiency of BPS should also be considered in its evaluation as a BPA substitute.'

observational supports low Cross-sectional in 54 female Korean cashiers with a one-week gloves/no-gloves crossover on EXPOSURE only; the outcome (fasting insulin, insulin resistance) is a single-timepoint association with no causal direction and no adjustment reported in the abstract. The only human study that both isolates receipt handling as the exposure and measures a metabolic outcome. Counted at observational tier, low quality. 'In cashiers without gloves, about a two-fold increase in urinary BPA concentrations was observed after work-shift.' 'Higher urinary BPA concentrations were associated with greater levels of fasting insulin and insulin resistance.'

2024 · Bulletin of environmental contamination and toxicology
in-vitro mixed low TEMPERED to mixed: the cytotoxicity and gene-expression changes appear at 113-143 µM, several orders of magnitude above any concentration a receipt could deliver through skin, so this is an in-vitro signal at a non-physiological dose and cannot carry a human-harm headline. Receipt extracts (BPS, diphenyl sulfone) screened on chicken hepatocyte spheroids: cytotoxicity only at 113-143 µM, orders of magnitude above any human dermal dose, plus gene-expression changes. In-vitro tier; low quality for a human-harm claim because the concentrations bear no relation to handling exposure. 'These chemicals elicited limited cytotoxicity with LC50 values ranging from 113 to 143 µM, and induced dysregulation in genes associated with lipid and bile acid homeostasis.'

observational mixed moderate Three volunteers; even intensive handling stayed within the non-occupational reference range. The comparison is against EFSA's 2015 temporary TDI, which EFSA's 2023 re-evaluation cut by roughly 20,000-fold; the dose conclusion depends entirely on which reference value is used. 'The calculated maximum BPA excretion per day after handling thermal paper was less than 0.2 μg/kg of body weight, suggesting a total daily intake over 25 times lower than the European Food Safety Authority's (EFSA's) proposal for a temporary tolerable daily intake (temporary TDI) (5 μg/kg/day).'

observational mixed moderate Isotope-labelled, six men. Establishes that dermal BPA is slower-clearing and more often unconjugated than dietary BPA — the mechanistic reason the route matters. Exposure, not outcome. 'After dermal exposure, cumulative excretion increased linearly for 2 days, and half the participants still had detectable urinary total BPA-d16 after 1 week.' 'Proportions of free BPA-d16 in urine following dermal exposure (0.71%-8.3% of total BPA-d16) were generally higher than following the dietary exposure (0.29%-1.4%).'
Bernier MR, Vandenberg LN
2017 · PloS one
observational mixed moderate 698 people observed. Real handling is longer and broader than EFSA's model assumes; modelled intake matches EFSA's own dermal estimate. 'individuals handle receipts for an average of 11.5 min, that >30% of individuals hold thermal paper with more than three fingertips, and >60% allow the paper to touch their palm.' 'the most conservative estimated intake from handling thermal paper in this population is 51.1 ng/kg/day, similar to EFSA's estimates of 59 ng/kg/day from dermal exposures.'
